Permission Issues

Camera permission denied

Symptoms: Student cannot start quiz, camera permission dialog doesn’t appear Solutions:
  1. Check browser camera permissions in settings
  2. Look for the camera icon in the browser address bar
  3. Reset site permissions and try again
  4. Ensure no other application is using the camera
  5. Try a different browser

Screen sharing permission denied

Symptoms: Quiz won’t start, screen share prompt doesn’t work Solutions:
  1. When prompted, select “Entire Screen” not a specific window
  2. Check browser permissions for screen sharing
  3. On macOS, check System Preferences > Security & Privacy > Screen Recording
  4. Try Chrome or Firefox for best compatibility

Multiple monitor detected

Symptoms: Quiz blocked with “multiple monitors detected” message Solutions:
  1. Disconnect external monitors
  2. On laptops, close external display lid
  3. Go to Display Settings and disable extended display
  4. Use “Duplicate” mode instead of “Extend” if a second monitor is needed

Storage Issues

Test connection fails

Symptoms: Cannot save Proctor Settings, “connection failed” error Solutions:
  1. Verify Access Key and Secret Key are correct
  2. Check bucket name exists and is accessible
  3. For Cloudflare R2, ensure Endpoint URL is set
  4. Verify the credentials have read/write permissions
  5. Check for typos in the configuration

Snapshots not uploading

Symptoms: Snapshots show “Failed” status, missing captures Solutions:
  1. Check storage configuration in Proctor Settings
  2. Verify internet connectivity
  3. Check browser console for error messages
  4. Ensure bucket has sufficient space
  5. Verify CORS is configured on the bucket

Presigned URL errors

Symptoms: “Failed to get upload URL” errors Solutions:
  1. Re-test storage connection in Proctor Settings
  2. Check that credentials haven’t expired
  3. Verify bucket policy allows presigned URL generation
  4. Check server logs for detailed error messages

Quiz Issues

Proctoring not activating

Symptoms: Quiz starts without proctoring, no permission dialog Solutions:
  1. Verify proctoring is enabled on the specific quiz
  2. Check that proctoring is globally enabled in Proctor Settings
  3. Ensure the proctoring patches are installed
  4. Clear browser cache and try again

Quiz terminated unexpectedly

Symptoms: Student kicked out of quiz mid-session Possible causes:
  1. Anomaly action set to “Terminate Quiz”
  2. Max warnings exceeded
  3. Browser crash or tab close
  4. Network disconnection
Review steps:
  1. Check the submission for termination reason
  2. Review anomaly log for what triggered termination
  3. Decide if student should be allowed to retake

Fullscreen keeps exiting

Symptoms: Student repeatedly prompted to return to fullscreen Solutions:
  1. Don’t press Escape during the quiz
  2. Keep mouse within the browser window
  3. Avoid accidentally pressing F11
  4. Check for popup notifications causing focus loss

Face Detection Issues

False positives - “No face detected”

Symptoms: Anomaly triggered when face is visible Solutions:
  1. Improve lighting - face should be well-lit
  2. Adjust camera angle - face should be centered
  3. Increase “No Face Threshold” in settings
  4. Remove items blocking the face (hats, masks)
  5. Clean the camera lens

False positives - “Multiple faces”

Symptoms: Anomaly triggered when alone Solutions:
  1. Check for posters/photos with faces in background
  2. Remove reflections showing additional faces
  3. Increase “Multiple Face Threshold” in settings
  4. Ensure adequate distance from camera

Face detection not working

Symptoms: No face-related anomalies ever triggered Solutions:
  1. Verify face detection is enabled in Proctor Settings
  2. Check that webcam capture is enabled
  3. Ensure the face detection model is loading (check browser console)
  4. Try a different browser

Video Recording Issues

Recording not starting

Symptoms: No video available after quiz submission Solutions:
  1. Verify video recording is enabled in Proctor Settings
  2. Check storage has sufficient space
  3. Ensure screen sharing permission was granted
  4. Check browser console for MediaRecorder errors

Chunks failing to upload

Symptoms: Recording shows incomplete, missing chunks Solutions:
  1. Check internet connection stability
  2. Reduce chunk duration for unstable connections
  3. Verify storage configuration
  4. Check for browser errors during upload

Video playback issues

Symptoms: Cannot play back recorded video Solutions:
  1. Wait for all chunks to finish uploading
  2. Check that recording status shows “Complete”
  3. Try a different browser for playback
  4. Verify presigned URLs are generating correctly

Safe Exam Browser Issues

SEB configuration won’t download

Symptoms: Clicking “Open in SEB” does nothing Solutions:
  1. Check browser popup blockers
  2. Try right-click and “Save As”
  3. Verify SEB is enabled in Proctor Settings
  4. Check browser console for errors

SEB won’t open the configuration

Symptoms: Downloaded .seb file doesn’t launch SEB Solutions:
  1. Verify SEB is installed
  2. Check .seb file association in system settings
  3. Try opening SEB first, then loading the config
  4. Reinstall SEB

Quiz doesn’t load in SEB

Symptoms: SEB opens but quiz page is blank or errors Solutions:
  1. Check that quiz URL is accessible
  2. Verify SSL certificate is valid
  3. Check SEB version compatibility
  4. Try regenerating the SEB config

Browser Compatibility

If issues persist

  1. Update browser to latest version
  2. Clear browser cache and cookies
  3. Disable browser extensions
  4. Try incognito/private mode
  5. Try a different browser

  1. Check Proctor Settings configuration
  2. Review server error logs
  3. Note the exact error message and steps to reproduce
  4. Contact your system administrator

For Students

  1. Note the error message
  2. Take a screenshot if possible
  3. Contact your instructor with details
  4. Include: browser version, operating system, time of issue
