Here are three complete journeys showing how a patient moves through the home care system from start to finish. Each journey shows which record is created, who creates it, and what happens next. *** ## Journey 1: New Patient, First Home Visit A family calls and asks for a home care nurse to visit their elderly mother who has just returned from hospital after knee surgery. ``` Family calls the clinic │ ▼ Care Coordinator registers the patient (Patient record created: name, address, date of birth, mobile number, medical conditions, current medicines, allergies) │ ▼ Home visit is booked (Patient Appointment created: date, time, nurse assigned, appointment type = Home Consultation) Confirmation SMS sent automatically to patient's mobile │ ▼ Day of visit: Nurse receives the day's visit list Nurse travels to the patient's home │ ▼ Nurse opens the visit record on their device (Patient Encounter created or opened) Reviews patient's history and the reason for the visit │ ▼ Nurse records vital signs (Vital Signs record: blood pressure, heart rate, temperature, oxygen level, blood sugar if diabetic) │ ▼ Nurse examines the patient Records findings in the visit record: - How the patient is feeling - Any complaints or new symptoms - Mobility level (can they walk, do they need help, are they bedridden?) - Feeding ability (eating on their own or needing help?) - Wound status if applicable │ ▼ Nurse submits the visit record (Patient Encounter submitted - becomes permanent record) Appointment status changes to Closed │ ▼ Care Coordinator creates the invoice (Sales Invoice: home visit fee charged) Payment collected │ ▼ Next visit scheduled ``` *** ## Journey 2: Ongoing Wound Care (Regular Visits) A patient has a non-healing leg wound and needs wound dressing changed twice a week. ``` Wound care visits scheduled for Tuesdays and Fridays (Two Patient Appointments per week, appointment type = Home Consultation, recurring for next 4 weeks) Reminder SMS sent 2 hours before each visit │ ▼ Nurse arrives at patient's home Opens the Patient Encounter for today's visit Reviews previous visit notes - how was the wound last time? │ ▼ Nurse takes a photo of the wound before starting (Observation created: Wound Photo - image attached) │ ▼ Nurse begins the wound dressing procedure (Clinical Procedure record opened: Wound Dressing (Home Visit)) System records the start time │ ▼ Nursing tasks completed step by step: 1. Remove old dressing 2. Clean wound with Normal Saline (100ml) 3. Apply Betadine Solution 4. Apply sterile gauze pads 5. Wrap with bandage roll 6.
