# Patient Management

> How to register patients, manage patient records, record medical history, and search for returning patients at the Diagnostic and Scan Centre.

Every visit at the diagnostic centre is linked to a patient record. Registering the patient correctly the first time saves time on every future visit and ensures test results and reports stay connected to the right person.

## Registering a New Patient

Go to **Masters** in the workspace sidebar and click **Patient**, then click **New**.

### Required Fields

| Field         | What to Enter                                              |
| ------------- | ---------------------------------------------------------- |
| First Name    | Patient's first name                                       |
| Last Name     | Patient's last name                                        |
| Date of Birth | Used to calculate age automatically                        |
| Sex           | Male / Female / Other                                      |
| Mobile        | Primary contact number for SMS confirmations and reminders |

### Recommended Fields

| Field              | What to Enter                                                        |
| ------------------ | -------------------------------------------------------------------- |
| Email              | For sharing reports digitally                                        |
| Blood Group        | Important for procedures requiring blood products                    |
| Allergies          | Any known allergies, especially contrast dye or medication allergies |
| Chronic Conditions | Ongoing conditions such as diabetes, hypertension, kidney disease    |
| Emergency Contact  | Name and number of a family member                                   |

### India Health ID (ABHA)

If the patient has an ABHA (Ayushman Bharat Health Account) card, record it here:

| ------------ | ----------------------------------- |
| ABHA Number  | 14-digit ABHA number                |
| ABHA Address | Health ID (e.g., name\@abdm)        |
| ABHA Card    | Upload a photo of the physical card |

Click **Save**. The system assigns a unique Patient ID automatically.

## Finding a Returning Patient

For patients who have visited before, search by name or mobile number before creating a new record.

1. Go to **Masters** and click **Patient**.
2. Use the search bar and type the patient's name or mobile number.
3. Click the patient's name to open their record.

Alternatively, when booking an appointment, type the patient name in the Patient field and the system will search existing records as you type. If the patient does not appear, click **Create New** from within the appointment form.

## Viewing a Patient's Full History

From any patient record, click the **Patient History** button in the top action bar. This opens a timeline of every investigation the patient has had at the centre:

* Appointments (with dates and types)
* Scan procedures and their status
* Lab tests and results
* Diagnostic reports
* Billing invoices

## Editing a Patient Record

Open the patient record and click **Edit**. You can update:

* Contact information (new mobile number, address)
* Allergy information (add newly discovered allergies)
* Chronic conditions
* ABHA details

Click **Save** after making changes.

| Blood Group        | Displayed on all clinical records                     |
| Allergies          | Alert displayed when booking procedures               |
| Medical Conditions | Background health context for the radiologist         |
| Emergency Contact  | Called in case of adverse reaction during a procedure |
| ABHA Number        | National health ID for ABDM-linked centres            |
| ABHA Card          | Upload of physical ABHA card                          |

## Best Practices

* Always search for the patient before creating a new record. Duplicate patient records cause report history to be split.
* Record contrast dye allergies accurately. A patient with a contrast allergy must be flagged before any CT or MRI with contrast is performed.
* Collect the mobile number for every patient. Without it, the appointment confirmation and reminder SMS will not be sent.
* Record the referring doctor's name in the appointment or patient record so the Diagnostic Report can be addressed correctly.
